SERVICE TERMS & BILLING NOTES

All rates are one-way, per patient, per trip unless otherwise stated.

Mileage is billed based on loaded miles only (patient in vehicle).

Trips exceeding 120 miles require advance approval and are quoted individually.

Waiting time is billed when delays are caused by facility readiness, patient availability, or extended transfers beyond scheduled pickup time.

After-hours, weekend, and holiday rates apply based on actual pickup time.

Bariatric services apply when patient size, weight, or medical needs require specialized equipment, reinforced stretchers, or additional crew for safe transport. Additional crew or special handling may be billed when required for patient safety or regulatory compliance.

Same-day cancellations (within 2 hours of scheduled pickup) may be billed at 50% of base rate.

No-shows where the transport team is on site may be billed at 100% of base rate.

Contracted facilities may qualify for volume-based or flat-rate pricing upon agreement.
